We would like to let you know hat UCB Pharma, Inc. has acquired Celltech Pharmaceuticals Inc. and Upstate Pharma LLC. The product lines formerly sold by Celltech and Upstate will follow the same UCB Pharma return process outlined below. We appreciate your business and look forward to a continued profitable relationship. UCB Pharma, Inc. is pleased to announce that effective July 1, 2005 we will join the Universal Solutions, Inc. One Box Return program. This means, that for those of you who are currently using the One Box Return include UCB Pharma, Celltech Pharmaceuticals, Inc. and Upstate LLC returnable products in your One Box Return shipment. UCB Pharma has chosen the One Box Return program since it is not only cost effective, but also very simple. We will pay for all charges associated with your return, except for shipping. All you need Pharma, Celltech and Upstate returnable products in your current One Box R. As an indirect account, you will receive a check or credit from your wholesaler for returns that qualify. For your convenience, we have enclosed a copy of our Returned Goods Policy. Partnering with One Box Return is another example of UCB's offering the best possible service to our customers. We are confident you will be pleased with the services provided by One Box Return. Please see the instructions below. If the following instructions are not adhered to we cannot guarantee proper credit will be issued for your return. 1. Contact USI prior to returning product to obtain a RAF (return authorization form) number, and label. They can be contacted via phone or website, please see below. **All packages must be shipped with a USI preauthorized shipping label.** 2. Your assigned debit memo number or whichever number is used for tracking and reconciliation purposes must be listed on the USI authorized label on the debit memo line. 3. Individual debit memos should be boxed and identified separately. 4. Include the RAF (return authorization form) along with a packing slip and/or debit memo with your return. NOTE: If shipping multiple boxes, please place a copy of the RAF on the outside of each Box!!
