PHARMACY NETWORK UPDATE

The meeting with Outlook Graphics, Jabas and RESTAT was delayed from last week to yesterday (Friday), The meeting went extremely well and we are scheduling a second meeting to siscuss implementation of the prescription benefit program. Outlook has over 1,000 plan members and is currently considering the co-pay differential option. This plan will increase the members co-pay by $5.00 if they use an out of network pharmacy in the Fox Valley. Attatched to this e-mail is a list of participating pharmacies (please check your information for accuracy). Network pharmacies will be expected to engage in utilization interventions and patient outcome practices. The success of the network is directly linked to the value we can provide in utilization and outcomes management. Otherwise, we are no different than any other program onthe street. If for any reason you so not wish to participate, please advise ASAP.

I boiled the name of our group down to "Valley Independant Pharmacy Alliance". The first part "Valley" gives us some geographic scope without pigeon holing us strictly the Fox Valley. The second part "Independant Pharmacy" clearly identifies who we are. The third part "Alliance" I think communicates what we have accomplished by working together to serve patients and businesses in our communinity. I choose not to use network because it sounds to PBMish. Things are starting to role...more to come!

Warning: An Incorrect Substitution May Pose a Treat to Patients

It has been brought to my attention that some patients who were prescribed NEORAL® (cyclosporine, USP) MODIFIED have had their prescriptions inappropriately substituted with a  non-bioequivalent product from Apotex Corporation (cyclosporine, USP). These products are not therapeutically equivalent and consequently cannot be automatically substituted for each other.

According to the FDA-approved product label, NEORAL® and SANDIMMUNE® (cyclosporine, USP) are not bioequivalent. These drugs, and their respective equivalents, cannot be automatically substituted for each other. NEORAL® prescriptions that are substituted at the pharmacy level with the Apotex product without appropriate drug level monitoring place patients at risk of cyclosporine underexposure with potentially serious clinical consequences.

Inappropriate substitution poses a genuine threat to transplant patients, so I ask for your help to ensure that no patient is put at risk. When you receive a script for cyclosporine, please take the following action to ensure that the product is accurately dispensed.

  • VERIFY with the patient's health care provider the cyclosporine formulation that is being requested. If the script is for NEORAL®, then SANDIMMUNE® or its generic equivalents should not be substituted

Enclosed is a sheet to help you differentiate cyclosporine formulations.

Your vigilance in this matter is greatly appreciated.

Sincerely yours,
Mark D. Grebenau, MD, PhD
Director, Medical Information and Communication
Transplantation Business Unit

NEORAL® and Sandimmune® Soft Gelatin Capsules and Oral Solutions are indicated for the prevention of organ rejecting in kidney, liver, and heart allogeneic transplant recipients. NEORAL (cyclosporine, USP) MODIFIED and SANDIMMUNE® (cyclosporine, USP) are not bio-equivalent and cannot be used interchangeably without careful monitoring of cyclosporine blood concentration and dosage adjustments based on individual response. The principal adverse reactions of cyclosporine therapy in transplantation are renal dysfunctions, tremor, hirsutism, hypertension, and gum hyperplasia. NEORAL® and SANDIMMUNE® are systematic immunosupressants and may increase the susceptibility to infection and the development of neopolasia.

Attn: Lilly Buyer

RE:ZYPREXA Zydis (olanzapine) Orally Disintegrateing Tablets-Updated Stocking Information

We wish to inform you that on June 6, 2003 Lilly will begin supporting ZYPREXA Zydis, the orally disintegrating form of ZYPREXA (olanzapine), with an extensive marketing campaign to community-based psychiatrists and community mental health centers. Until now, ZYPREXA Zydis has primarily been used in institutional and long-term care facilities. The additional marketing efforts to community-based healthcare professionals will be in conjunction with current marketing activities in these inpatient care settings.

ZYPREXA Zydis is available in 5 mg, 10 mg, 15 mg, and 20 mg and is indicated for the treatment of schizophrenia, for maintenance of treatment response in schizophrenia, and for acute bipolar mania. ZYPREXA Zydis offers an alternative dosing form for patients who may be unable or reluctant to swallow regular tablets. We anticipate an increase in your current demand for all strengths of ZYPREXA Zydis.

Initial Stocking Orders:

Stocking incentives may be offered through your wholesaler. Please contact your wholesaler as soon as possible to discuss this opportunity.

Sales Forecast:

To assist you with your initial stocking quantities, Lilly anticipates that one ZYPREXA Zydis 10 mg 30-count Dose Pack should be stocked in pharmacies purchasing high volumes of ZYPREXA 10 mg tablets. Additionally, Lilly anticipates increased utilization of all four ZYPREXA Zydis strengths.

Important Safety Information for ZYPREXA:

In short-term schizophrenia and bipolar mania trials, the most common treatment-emergent adverse event associated with ZYPREXA was somnolence. Other common events were dizziness, weight gain, personality disorder (COSTART term for nonaggressive objectionable behavior), constipation, akathisia, postural hypotension, dry mouth, asthenia, dyspepsia, increased appetite, and tremor. A small number of patients experienced asymptomatic elevations of hepatic tranaminase; none of these patients experienced jaundice. Prescribing should be consistent with the need to minimize the risk of tardive dyskinsia, seizures, and orthostatic hypotension. Phenylketonurics should be advised that ZYPREXA Zydis contains phenylalnine.

For Prescribing information on ZYPREXA Zydis, please refer to www.ZYPREXA.com or call 1-800-545-5979.

Very truly yours,

Eli Lilly and Company

Re: Official complaint of Unfair Business Practices and Severe Client Confidentiality Violations by Meerdink & Associates

Dear President and Members of the Greenville Market Condo Association:

As a member of the Greenville Market Condo Association (GMCA) of the Town of Greenville Wisconsin, it is my duty to file this official complaint to the GMCA Board to bring attention to the numerous unfair business practices against RxLink Pharmacy by the Managing Real Estates firm: Meerdink & Associates, Inc. and Russell Meerdink himself.

Greenville Pharmacy by RxLink opened its doors in 2001 in Greenville, Wisconsin to serve the Greenville Community and the surrounding communities.

This written complaint will indicate the numerous unfair business practices by Meerdink & Associates and Russell Meerdink.

The Real Estates Management firm (Meerdink & Associates, Inc. managed by Russell Meerdink), has commited numerous unfair business practices, which have caused irreparable damage to RxLink Pharmacy.  The unfair business practices are now being reported to the proper State and Federal Agencies for review.
